Bus route (e.g. Route 9 - Lambton College, or Care-a-Van)
Location and direction of travel
Date and time of incident
Bus number, if possible (3-digit number located inside the bus just above the driver)
Description of the driver
Your name and telephone number
A report is filed based on the information we receive. The incident will be investigated from one of our office staff. If you would like a follow-up phone call or email, please indicate this at the time the incident is filed.
